Who Are We?
Under the hospice Medical Director’s supervision, a team of clinical professionals will work in communication with our patient’s primary care physician to provide physical, clinical, spiritual, and social care and support to our patients and their families. Our care team includes:
- Physician
- Nurse Practitioner (NP)
- Registered Nurse (RN)
- Licensed Practical Nurse (LPN)
- Certified Nurse Assistant (CNA)
- Social Worker
- Chaplain
- Volunteers
- Clinical PharmD resource
- Physical Therapy (limited)
- Dietitian
Level of care
SympathyCare hospice provides multiple levels of care based on the patient’s need. The levels of care are Routine Home Care, Continuous Home Care, General Inpatient Care and Respite Care.
Routine Home Care
Most common level of care in hospice. Care is provided in patients’ homes, nursing facilities, residential or assisted living facilities. Patient is generally stable and the patient’s symptoms, like pain or nausea and vomiting, are adequately controlled.
Continuous Home Care
Crisis-like level of care for short-term management of out of control patient pain and/or symptoms and is usually provided in the home setting.
General Inpatient Care
Usually provided outside the home, in an inpatient setting at a medical facility like a hospital or skilled nursing facility. This is a level of care for short-term management of out-of-control and crises-like conditions of patient pain and/or symptoms.
Respite Care
This level of care is tied to caregiver needs, not patient symptoms. Up to 5 days, Respite care can be provided in a Skilled Nursing Facility or a Hospice Inpatient Facility to allow family member or friend who’s the patient’s caregiver can take some time off.

Who pays for hospice:
Hospice is paid for entirely services includes hospice care, medications, medical equipment, medical supplies related to hospice diagnosis by Medicaid, Medicare, VA, Private Insurances, or other sources. If a patient does not have Insurance, SympathyCare Hospice will cover for that patients’ needs based on the terminal illness for which they are admitted.
